This booking form is for Australians seeking an appointment to lodge an application for an Australian Passport for children bwtween 16 amd 17 years of age. Before making the booking, please make sure you have completed the PC8 online form which can be found at https://www.passports.gov.au/

Use this form if your child is:
- between the ages of 16 and 17; and
- an Australian citizen

Once you have completed the online form, you should:
- confirm the preferred date and time of your lodgement interview;
- print and sign the PC8 form from your passport account;
- provide three (3) new Australian Standard sized photos*;
- present original documents confirming your child's identity;
- present the passports of all people with parental responsibility for the child; and
- pay the application fee.

Some foreign authorities require DFAT to issue your document with an Authentication Certificate. We call this "legalisation services".

Legalisation describes the process where DFAT verifies the signature, seal or stamp on an original Australian public document. When DFAT confirms the signature/seal on your document matches our records, we will issue the Authentication Certificate. Then you can present the document to your foreign authority.

Authentication (single page document)

This booking form is for Australians seeking an appointment to complete an adult passport renewal. Before making the booking, please make sure you have completed the PC7 online form which can be found at https://www.passports.gov.au/

Use this form if you are:
- aged 18 years or over; 
or
- have a passport which was issued when you were 16 or older;

and
- you are an Australian citizen;
- your passport has been expired for less than three years;
- your old passport shows your current name, date of birth and sex.

Once you have completed the online form, you should:
- confirm the preferred date and time of your lodgement interview;
- print and sign the PC7 renewal form from your passport account;
- provide three (3) new Australian Standard sized photos*;  
- present your old Australian Passport at interview;
- present original documents confirming your identity; and
- pay the application fee.

- an Australian citizen
- aged 18 years or over - and this is your first adult passport
- your passport has expired for more than 3 years
- unable to use the PC7 renewal application

Passport Application (Adult)

If you have an Australian document that requires signature by an official witness, Australian Consular Officers can usually perform this role (check the requirements of your document before booking an appointment). 

Use this booking form to arrange for us to witness a signature on an Australian document, such as a Statutory Declaration. 

Please note that we are only able to act as witnesses for Australian documents (for both domestic and overseas use). We cannot act as witnesses for foreign documents.

Witnessing Signature (Statutory Declarations and other Australian Documents)

In some countries, foreigners must provide evidence that they are free to marry, before a legal marriage ceremony can be held in that country. The CNI is a general statement of the law in Australia concerning the recognition of foreign marriages. It states that the person named in the certificate has signed a declaration and, on the basis that the declaration is true and correct, there is no legal reason why they cannot marry.

The Certificate also states that we are unable to confirm whether the person named is actually free to marry. This is because there is no central marriage registry in Australia, and we are not able to check or confirm that the person is not already married. A CNI is not a requirement under Australian law. 

The client must check whether or not their CNI must be issued in that country, or if a CNI issued in Australia or a third country is acceptable, with the government authorities where they intend to marry. Rules can vary in different parts of the same country. Because of this, the client should also be encouraged to check with the government authorities in the state or province where they intend to get married.

You need to submit the following documents with your application.

Applicant:
- Australian passport
- Foreign passport with evidence of permanent residence in Australia
(If you can't provide an Australian or foreign passport, you will require cardinal identity documents (such as a birth certificate, citizenship certificate or ID card). You must supply at least one form of photo identification.)
- Divorce Certificate for previous marriage (if applicable)
- Former partner's Death Certificate (if applicable)
- If under 18 years of age, copy of the judicial authorisation to marry issued by an Australian court of law

Partner:
- Australian passport, or
- Foreign passport
(If you can't provide an Australian or foreign passport, you will require cardinal identity documents (such as a birth certificate, citizenship certificate or ID card). You must supply at least one form of photo identification.)

Documents can be originals or certified as true copies by an authorised local provider. Documents not in English need to be accompanied by a certified translation. In Australia, the translation needs to be prepared by a translator certified by the National Accreditation Authority for Translators and Interpreters (NAATI). Overseas, translations need to be prepared by a translator recognised by the Australian Embassy, High Commission or Consulate.

Certificate of No Impediment (CNI)

This booking form is for Australians seeking an appointment to lodge an application for a new Australian Passport. Before making the booking, please make sure you have completed the PC7 or PC8 online form which can be found at https://www.passports.gov.au/

Use this form if you are:
- aged 75 years or over; and
- an Australian citizen

You should use the PC7 renewal form if your most recent Australian passport expired within the last three years. You should use the PC8 form if your last Australian passport expired three or more years ago, or you are applying for a passport for the first time. 

Once you have completed the online form, you should:
- confirm the preferred date and time of your lodgement interview;
- print and sign the PC7 or PC8 form from your passport account;
- provide three (3) new Australian Standard sized photos*;
- present original documents confirming your identity; and
- pay the application fee.

Please note you can request a 10 year validity on the Passport Application form instead of the standard 5 year validity.

Senior Passport Application (75+)

The Australian Embassy can witness signatures on and endorse photos for drivers' licence renewal forms, but does NOT renew the licences. 

Please contact the relevant road authority in Australia to obtain the necessary forms for renewing your licence while residing overseas. 

Clients are encouraged to check with the relevant road authority in Australia if sending their old licences back to Australia would allow renewals to be completed without engaging the Embassy.

The application must be lodged in person by at least one person with parental responsibility, and the applicant must sign the application if he/she is 10 years old and over.

Apostille (single page)

An affidavit is a legally binding statement of fact used as evidence in court or other legal proceedings. The person making the affidavit must swear an oath (a religious statement) or make an affirmation (a non-religious statement) that the contents of the affidavit are true and correct. It is an offence to swear or affirm false statements.

DFAT's role is to establish the identity of the person making the affidavit and administer the oath/affirmation. We do not endorse or rely on the content of the affidavit.

Given the importance of an affidavit, you need to make sure you have confirmed with your legal representative, or receiving authority, if there are special instructions for how we will administer your affidavit. It is important to make sure you are requesting the correct service from us.

If an affidavit contains attachments or is multiple pages, DFAT will bind the pages together as a general rule. Please confirm with your legal representative, or receiving authority, whether the binding of the pages together will mean your affidavit is not legal.
